Due to the fact that tooth preservation is often desired nowadays, even at an advanced age, the post-endodontic treatment of severely damaged teeth is of particular importance. In this respect, post restorations are an established treatment method whose clinical success, including relevant complications at the post level, has already been investigated many times. However, only a few studies address the treated tooth itself. Therefore, in this retrospective longitudinal study, the extraction of the post-bearing tooth was defined as the primary target event. A survival time analysis was used to gain additional knowledge regarding the survival time of the treated teeth. In addition, influencing factors and the reasons for a necessary extraction were analyzed. The data included in the study came from a total of 735 patients who were treated at the Polyclinic for Dental Prosthetics at Justus Liebig University Giessen between 2004 and 2022. A total of 1053 individual cases were recorded and analyzed using the Kaplan-Meier method. Within the observation period of up to 18 years, a total of 245 teeth (23.3%) had to be extracted. The average survival time of a post-retained tooth was 11.74 years. The 5-year survival rate was 79.9% and the 10-year survival rate was 58.98%. In the univariate comparison using the log-rank test, the factors patient age, material, bone level and prosthetic restoration were detected as statistically significant influencing factors (p < 0.05) on the survival time. Similarly, the same factors also had a significant influence (p < 0.05) on the time to tooth extraction in the Cox model. The indication for the extraction of a post-bearing tooth was a tooth/root fracture in 32.24% of cases. Other causes were secondary caries (18.78%), an apical inflammatory process (18.37%), periodontal attachment loss (16.73%) or changes in the prosthetic planning (6.53%). In some cases (7.35%) there was a different or unknown cause. Fractures occurred mainly within the first five years after post insertion and especially when the prosthetic restoration was a telescopic prosthesis. This can be seen as an indication to avoid post-supported teeth as abutments for telescopic prostheses.
