Objective: The aim of the present retrospective study was to investigate the clinical performance of post and cores over a period of up to 16 years (2004-2020) and to identify possible influencing factors. Additionally, the outcome of changes in the de-cision-making process for post and cores was to be evaluated.
Material and method: Data from 653 patients at the Department of Prosthodontics at the Justus-Liebig University of Gießen, who received a total of 953 post and cores, built the basis of this study. Indirectly fabricated posts were cast from high gold con-taining alloy or a non-precious metal. Fibre-reinforced posts were used in direct man-ners. The post and cores functioned as abutments for single-crowns, bridges and pri-mary crown retained removable partial dentures (telescopic dentures). Further to these aspects, age, gender, localisation, luting material, bone level at cementation and prac-titioner were investigated. The Kaplan-Meier analysis was used to represent the sur-vival time. The target events were the clinical success – time until first complication and the clinical survival – functional period of post and cores excluding reversible complications. Statistical correlations were mapped by the log-rank test and multifac-torial Cox regression.
Results: The mean survival time of the post and cores was 9.54 years. The 5-year and 10-year survival rates of all post and cores were 69.9% and 48.1% respectively. The log-rank test found significant differences for the parameters type and localisation of tooth, type of covering prosthetic restoration, post and core material and bone attach-ment. The multifactorial Cox regression showed significance for higher age at post and core cementation, covering prothetic restoration and bone attachment.
Conclusion: Post and cores have a good, long-term prognosis in the university clini-cal setting, irrespective of used materials, luting agents and localisations. However, it is reduced by the inclusion of post and cores in telescopic dentures and by the restora-tion of periodontally (pre-)damaged teeth. In these cases, the indication of a post and cores should be more stringent. To minimize complications, patients with post-retained teeth should participate in an intensive recall programme.
